The Foundation for Mental Health (FMH) is a nonprofit organization that provides permanent, stable supportive housing to homeless individuals with mental health needs.  FMH serves Grand Traverse, Leelanau, Kalkaska, Benzie and Antrim Counties and uses state and federal subsidies as well as private funds to drive its mission.  If individuals wish to be considered for housing with FMH, they must be referred by a case worker or similar service provider (forms available online or via fax/phone call).  In order to qualify for FMH programs, an individual must be homeless (under federal definition), mentally disabled (as certified by a clinician) and have little or no income. FMH provides up to 100% leasing subsidy and pays utilities if an individual is unable. FMH is the largest provider of supportive housing in the Grand Traverse area, please call/email with any questions.
